Quality Improvement Plan

A Quality Improvement Plan (QIP) is all about improvement. It provides an opportunity to focus on how and what to improve for better patient-focused care. As such, it is a unique document, designed by, and for, PSFDH.

The QIP drives change by formalizing a plan and facilitating shared dialogue to support continuous quality improvement processes. The QIP also provides a means to communicate accountability and transparency to patients, staff and our community.

PSFDH is focused on creating a positive patient experience and delivering high quality and safe health care.
