Perth and Smiths Falls District Hospital offers a wide variety of services to
meet your health care needs. The following healthcare services are
provided by family practitioners, associate family practitioners, nursing
staff and dedicated clinical and support professionals.

Acute Care Services
Including general
medicine, internal medicine, general surgery, dental surgery, ENT,
orthopaedics, urology, obstetrics, gynecology, anaesthesiology, and
pediatrics on an in-patient and out-patient basis. Acute Care
Services include the following departments.
Medical and Surgical Department
The Medical and Surgical Unit at the Perth
and Smiths Falls District Hospital is located on the second floor at both
sites. There is a kitchen available for patient use and a solarium
for both patients and visitors. A resource nurse is in charge to
answer any questions or concerns of patients and their families.
There are sixty-three medical and surgical beds available between both
sites, comprised of ward, semi-private and private accommodation. Each
room has a complimentary telephone. A pay telephone is on the floor, if
needed. Television services are available for purchase.
Visiting hours:
- from 1 p.m. to 8 p.m. daily
- two family members are allowed to visit per day - we ask that they remain a short time only (30 - 45 minutes)
- exceptions are made for compassionate reasons - critical or palliative patients
- visiting arrangements are at the discretion of the patient's physician or nurse on duty.
Care is provided for a wide range of
medical conditions and surgeries.

Departments, one in Perth and one in Smiths Falls.

Community Care Access Centre
The
South East Community Care Access
Centre is charitable, not for profit corporation, fully funded by
the Ministry of Health and Long Term Care and does not charge for its
services. It is the sole access point for Home Care and Placement
Coordination in Lanark, Leeds and Grenville. Services are available to all
ages, from newborns to the elderly and are available in home, school,
workplace and long term care.

Pastoral Care
Pastoral Care provides spiritual guidance
and support. Clergy of all denominations visit the hospital regularly. A
Chapel Service is conducted every Friday at 11 a.m. at the GWM site and is
open to all patients. Even if you do not attend a church, a minister of
your choice will be pleased to call on you. Please speak to the nurse if
you wish a visit.

Cardiopulmonary Services
Comprised of Cardiology Techs and Registered
Respiratory Therapists, the Cardiopulmonary Department assists the health
care team with the diagnosis and treatment of acute and chronic cardiac
and pulmonary disorders.
The following services are available:
Cardiac Services:
- Electrocardiograms (ECGs or
EKGs)
- 24 and 48 hour Holter
monitoring (portable heart monitors)
- Event recording (portable heart
monitors worn for up to 2 weeks)
- Ambulatory blood pressure
monitoring (for 24 hrs)
- Stress (treadmill) tests
- Pacemaker checks
Pulmonary Services:

Spirometry (basic lung function test), before &
after bronchodilator with inhaler instruction if needed

Complete Pulmonary Function Tests (PFT), including
spirometry, lung volumes, and single breath diffusion

Methacholine challenge testing (asthma provocation)

In patient services
for patients who no longer require acute care, but who are unable to go
home or receive care in a long term care centre, are available at the
GWM site. Admission to these specialized care beds is based on an
in-depth assessment of patient needs.

Clinical Nutrition
Services
Good nutrition during your hospital stay
and after discharge can be an important key to your recovery and health. A
Registered Dietician is available to discuss your individual nutritional
needs.
Outpatient dieticians services are
available at both sites with a doctor's referral.
A two day Diabetic Education Program is
held monthly at the GWM site. A doctor's referral is necessary and both
client and family members are encouraged to attend. There is a small fee
to cover the cost of materials.
Nutrition and Food Services
Patient meals are served at the following
times:

a.m. - 8:00 a.m.

Lunch 11:30
a.m. - 12:00 noon
·
Dinner 4:30
p.m. - 5:00 p.m.
Visitors are welcome to use the cafeteria.
Hours of operation are 7:30 am to 1:30 pm Monday to Friday. In addition,
vending machines are available for light snacks and drinks at all times.

Clinical Laboratory Services
The Perth and Smiths Falls District Hospital is well
served by comprehensive, clinical laboratory services at both sites,
including Chemistry, Hematology, Transfusion Medicine and Microbiology. We
perform laboratory tests for inpatients, emergency room patients and all
pre-op patients. Using the latest technology,
and with highly trained and certified staff; timely, accurate results are
provided confidentially to your physician. As well, we work in partnership
with the Hospitals In-Common outpatient Laboratory in Perth for the
provision of laboratory services to private physicians and clinics.
In order to be registered for your
hospital/laboratory visit, you must bring your requisition, your hospital
card and a valid Ontario health card to the hospital/laboratory. We need
to be able to verify who you are in order to collect the required
specimens and perform the testing. Remember, your physician depends on
your laboratory results to make accurate diagnosis and treatment plans.
In fact, up to 85% of these decisions are based on your laboratory
results.
You also play an important role in ensuring the
quality of your lab results. Medications, alcohol, nicotine, caffeine,
food and exercise can all affect your laboratory results. Whenever
possible, be prepared. Ensure you understand what is required for the
specific lab test you are having done. The Canadian Society of Laboratory

Infection
Prevention & Control Service
Vision
The Infection Prevention and Control
Program of the Perth & Smiths Falls District Hospital will strive to
protect and promote the safety and well-being of patients, staff members,
volunteers and visitors from exposure to multi-drug resistant organisms
and preventable healthcare-acquired infections in a fiscally responsible
manner.
This is accomplished through four
fundamental activities:

Surveillance
Monitor and investigate exposures and outbreaks of potentially serious
infectious diseases. Early detection and containment is essential in
proper infection control.

Education
The Infection Prevention & Control Program will serve as an expert
resource. We provide on-going education to hospital personnel and
students, including hospital orientation, in-service training, and house
staff and other student education.

Consultation
The Infection Prevention & Control Program will provide expert,
evidence-based guidance on minimizing the risk of infection. As well,
consultation on infection prevention and control issues is offered to
external partners such as community agencies, Public Health and the
community at large.

Policy and Procedure Development
The Infection Prevention & Control Program will develop, and collaborate
on the development of policies and procedures that are evidence-based,
best practices or consensus based with the goal of providing appropriate
guidance in the management of infections or to prevent the acquisition
or exposure to infection in all areas of the organization.

comprised of qualified infection prevention & control professionals with both a nursing
and medical laboratory background.

Maternity Care
Services
On Maternity we have four Birthing Rooms
which are classified as LDRP rooms, meaning Labour, Delivery, Recovery and
Post Partum. Each Birthing Room has a jacuzzi tub, private bathroom and
electric beds. Partners are welcome to stay overnight as we also have
chairs that pull out into beds. Family and friends may visit and use our
Family Room which provides comfortable seating for adults as well as
children and includes a TV, microwave, coffee pot, kettle and fridge with
refreshments.
We encourage 24 hour rooming-in for our
new parents. This allows mom and dad the much needed exposure to their new
baby prior to being discharged from hospital. We work closely with Public
Health and following discharge, they will contact new mothers within 48
hours to make sure there are no problems. Public Health also provides
Lactation Consultant services for breast feeding mothers. We are also
associated with Connections, an organization that "connects" expectant
mothers with a support person to follow them throughout their pregnancy.
The Hospital has one obstetrician and a
number of General Practitioners on staff who deliver babies in Smiths
Falls. Our qualified Nursing Staff provide labour support, recovery and
post partum care with an emphasis on teaching of newborn care during the
post partum period. We deliver approximately 300 babies per year in Smiths
Falls.

Gynecology Referrals & Obstetrical
Referrals
Dr. Chan and Kerner will be accepting gynecology referrals
effective July 16th at fax number 613-283-0048.
For obstetrical referrals:
Effective August 1st our complement of obstetrical providers will
include Drs Chan and Kerner. We will continue to run the prenatal
clinic and share call and deliveries amongst the five physicians (Kerner,
Chan, Yunker, Muldowney, McGregor).
In order to facilitate what we anticipate to be an increasing volume of
obstetrical referrals, we are making a slight change. You may refer
your patients initially to the offices of any one of the 5 providers (fax
numbers below). If they are less than 36 weeks GA we will see them in our
offices and are happy to do shared care with those physicians who are
comfortable doing prenatal care until 34-36 weeks.
At 36 weeks we will arrange for them to be seen in our prenatal clinic
where they will meet the other providers.
We appreciate your referrals and would like to ask you to advise your
patients that the physician they are initially referred to may not be the
delivering physician and could be either an obstetrician or a family
doctor. We will be working together on call as we feel this shared
model is key to retaining and sustaining our new and existing providers.
Higher risk patients will of course be managed appropriately and you may
direct your earlier referrals accordingly.
Dr. Yunker will continue to be a c-section provider and receive direct
referrals for elective repeat c-sections.

Radiology Services
Diagnostic x-ray testing is provided at
both sites of the Hospital. The department serves elective as well as
emergency patients 24 hours per day, 7 days per week.
Ultrasound Services
A full menu of ultrasound testing is
available at both sites of the Hospital. Specialized vascular and cardiac
ultrasound tests are booked through the Smiths Falls site.

Rehabilitation Services
Under the capable direction of our physiatrist, the Hospital is very proud
to offer exemplary in-patient and out-patient specialized rehabilitation
services. The program offers support to patients and their families for
diagnoses such as stroke and other conditions which require these
extensive programs. Components of the program include nursing,
physiotherapy, activity programming, clinical nutrition, occupational
therapy, speech-language pathology and day hospital. Consultant
physiatrist services enhance these programs.

consists of nurses who provide the following service to both in-hospital

Kingston Regional Cancer Centre (KRCC)

education, support and counselling to the client and family

specialist) to the monthly follow-up clinics

Intraocular Lens Measurement done by our technician. The measurement
tells us the necessary thickness of the lens. The measurement takes
approximately 5mins to complete.

There is a fee for the measurement of
$100.00. The fee can be paid at the Hospital’s Business Office located
on the first floor next to the front entrance.
Please Note:
If you have already had your measurement done please tell the technician
when you arrive. The measurement will need to be redone but you will not
be charged a fee.

Lanark County
Sexual Assault and Domestic Violence Program
The Lanark County Sexual Assault and
Domestic Violence Program is funded by the Ministry of Health through the
Perth and Smiths Falls District Hospitals for medical care and treatment.
The Program supports a comprehensive and multi-disciplinary response to
domestic violence and sexual assault. The Emergency Departments of the
Perth and Smiths Falls District Hospitals, Carleton Place Hospital and the
Almonte General Hospital provide specialized medical and nursing care to
victims of domestic violence and sexual assault. This includes immediate
crisis intervention, crisis counseling, safety planning, follow-up,
referrals, advocacy and forensic evidence collection. We recognize that
healthcare providers have a unique opportunity as one of the first and
possibly only avenues where a victim will seek assistance. We therefore
have a responsibility to provide appropriate and sensitive interventions
that recognize a victim’s right to self-determination.

below. These documents have been recently revised and contain information
that can be accessed by anyone seeking assistance and support in relation
to these abuse issues. The protocols also specifically outline the
responsibilities of a number of agencies involved with domestic violence
and sexual assault: Ontario Provincial Police, Perth Police Service,
Smiths Falls Police Service, Crown Attorney’s Office, Victim Witness
Assistance Program, Lanark County Interval House, Children’s Aid Society,
Victim Crisis Assistance and Referral Service, Probation and Parole, and
the Partner Assault Response Program.

sexual assault issues, there are many agencies that offer very specific

Vascular
Protection Clinic
The Vascular Protection Clinic (VPC) is a
hospital based, outpatient clinic that offers secondary prevention to
patients who have experience a TIA or minor stroke and, primary prevention
to those patients who have significant vascular risk factors. Patients are
referred to the VPC by the emergency room physician or by their family
physician. Patients receive assessments, advice and treatment plans from
an Internal Medicine Specialist, a Registered Nurse and a Dietitian. This
clinic operated at the GWM site, and offers its service to Lanark, Leeds
and Grenville Counties.
The Vascular Protection Clinic is a joint
partnership between the Health Care Network of Southeastern Ontario,
Coordinated Stroke Strategy, Kingston General Hospital and The Perth and
Smiths Falls District Hospital.
