Annual Accessibility Plan
For Perth & Smiths Falls District Hospital
Executive Summary
The purpose of the Ontarians with
Disabilities Act, 2001 (ODA) is to improve opportunities for people
with disabilities and to provide for their involvement in the
identification, the removal and prevention of barriers. To this end, the
ODA mandates that each hospital prepare an annual accessibility plan.
This plan for the year of 2008 - 2009 has been
prepared by the Accessibility Working Group of the Perth & Smiths Falls
District Hospital (hereinafter referred to as “PSFDH”). To summarize, the
report describes:

The measures that Perth & Smiths Falls District Hospital has
employed in the past (refer to appendices A and B,

The measures that PSFDH will take during the year 2008 - 2009 (to
identify, remove and prevent barriers for people with disabilities who
work in or use the facilities including patients, their family members,
staff, volunteers and members of the community, refer to appendices C).
This year, PSFDH committed itself to the
continual improvement of access to hospital facilities, policies,
programs, practices and services for patients, their family members,
staff, volunteers and members of the community with disabilities; the
participation of people with disabilities in the development and review of
its annual accessibility plans; and the provision of quality services to
all patients, their family members and members of the community with
disabilities.
The Accessibility Working
Group will continue to identify barriers that it will be focus on over the
next several years.
